Clicking and dragging a guide line from the horizontal ruler creates a horizontal guide line.
Pressing the Alt key, (option / Mac), will toggle the line from horizontal to vertical. The same thing happens with the Vertical Ruler guide line. The Alt key can be toggled on and off while positioning the line.
For myself I found it convenient to drag both horizontal and vertical guides lines from the ruler on top - horizontal.

Moto, great tip! I didn't know about using the Alt key to toggle the guide lines. It makes sense; one can toggle using this key in other areas of Elements. Living and learning!
